Notice of destruction of inactive Special Education records for students born in 1990, 1991, 1992, 1993

(SPS) - With this article, Sapulpa Public Schools has posted notice of the destruction of inactive Special Education records for students born in 1990 through 1993.

It is district procedure and practice to destroy all inactive Special Education records after six (6) school years.

However, 60 calendar days before they are to be destroyed, we are required to provide notification. After the 60-day time period, the records will be destroyed.

The records for Special Education students born in 1990, 1991, 1992, and 1993 will be destroyed after Sept. 7, 2018.

Please be aware that due to circumstances beyond our control such as fire, weather, flood, relocation, out-dated electronic systems, etc, many records may not be available.

If you would like to take possession of the records, please complete the form below and hand-deliver the completed form along with a photo ID to the Department of Special Services at the Washington Administrative Center located at 511 E. Lee Ave. in Sapulpa, between the hours of 8 a.m. and 4 p.m. Records cannot be faxed or mailed.

The records will be released to only those students who are 18 years or older. Legal guardians will be required to show proof of guardianship to obtain a student's records.
